SYMPTOM & EXPOSURE FLOWSHEET

SELF-REPORTING

Families are being asked to self-report if a student tests positive for COVID-19.  Please contact madisonms.attendance@seattleschools.org  or call (206) 252-9204 to report that your child is out sick. Masking at school is required only when someone returns day 6 after testing positive for COVID.  Mask should be worn through day 10.  The school has a limited number of rapid COVID-19 tests available to send home with a student if requested.

TESTING CONSENT

This school year, Seattle Public Schools (SPS) is also providing COVID-19 testing to students whose families give consent. 

All SPS families were sent an email from CIC Health on Wednesday, Sept. 28 with a link to a consent form. If you haven’t already, please fill out the form. All families are strongly encouraged to provide consent. Please contact sps-testing@cic-health.com with questions.
