MESSAGE FROM SCHOOL NURSE

Please remind your student(s) to pick up their medication on the last day they will be attending school this year. Remember to check backpacks for medication and forms on the last day of school!

  • Medications to treat life-threatening conditions such as epinephrine and inhalers for moderate to severe asthma must be picked up on the last day your student attends school.
  • Non-emergent medications such as Advil and Tylenol may be picked up anytime on or before the last day of school.

If you do not want your student to transport the medications home, please let Nurse Mindy know (206) 743-3142).

Will your student require medication for the next school year?  Seattle Public School requires an updated Medication Authorization form each year. Please bring the medication authorization form to your student’s healthcare provider this summer. A form will be attached to your student’s medication. It can also be found here, under Forms, click on “Authorization for Medication to be Taken at School”. Please have this form signed by your student’s healthcare provider before the first day of school. Once signed, the healthcare provider can fax it directly to the Madison School Nurse at (206) 743-3142. If your student(s) will be attending West Seattle High School, you can fax the form to Nurse Aimee Osland at (206) 743-3186.  

Paperwork for emergency medication such as epinephrine must be completed and received by the school nurse before the first day of school in September.
